Patients provide data about their medical history and disability using 3 common scientific scales. The scientific scales include 1)Quick DASH for disabilities of the arm, shoulder, and hand; 2)Modified Oswestry Low Back Pain Disability questionnaire 3)Lower Extremity Functional Scale (LEFS). Unlike many other apps, the PT Clinic Intake app is based on proven scientific methods. After a patient answers all questions, a report can be wirelessly printed or emailed for inclusion in medical records. This is the free version of the PT Clinic Intake app.
